Title: Case Manager - Supplemental Location: California-North State Service Area-Redding-Mercy Medical Center Redding Other Locations: null The Case Manager (CM) is responsible for assessing, planning, facilitating and advocating for options and services through a continuum of care from admission to discharge for assigned patients. REQUIREMENTS: Current California RN licensure 3 yrs nursing experience in an acute care hospital. BSN or MSN from an accredited nusing school Working knowledge of Microsoft Office suite and work related software PREFERRED: 1-2 years case management experience in an acute care. required Certification appropriate for the position preferred (CCM, ABQAURP or ACM).
